Recommendation Form: Download copies of this form for your references to complete and send directly to the school. Three recommendations must be submitted. A personal letter of recommendation may be sent along with the form.

Applicants with foreign education: If a candidate is basing his/her eligibility on a degree from a foreign university outside of the United States or Canada, the official transcript or copy must first be evaluated by an evaluation agency listed in the section entitled "Foreign Transcript Evaluation Agencies". Only evaluations from these listed agencies will be accepted. The official or notarized evaluation showing degree equivalency and a subject breakdown must be submitted with the application for certification examination. This document must be in our office prior to the first day of the examination period in order to determine your eligibility.

PLEASE NOTE: An official transcript of college credits must accompany all completed applications. Application processing begins in December with applications accepted until March 1st.   A personal interview is also required before admission. Class selection occurs on or after April 1st.
