The Toxicology section provides the following kits free of charge. To order any of these kits, call 608-224-6257. You can also call our toll free number, 1-800-442-4618, select kits from the list of options and then select alcohol and blood lead kits. Information you need to provide when you call is your name and phone number, the agency name, agency account number, and the number and kind of kits you need.

Kit #6 Lead in Blood - one kit provides enough supplies for 100 tests

Each specimen should be labeled with the patients name and accompanied by a completed Toxicology requisition form.

Kit # 33 Coroner Blood Toxicology

Each specimen should be labeled with the patient's name and accompanied by either Form MV 1238 - Coroner's Report of Motor Vehicle Death or Form C-92 IC - Toxicology Request Form. These forms must be requested separately.

These kits are for law enforcement agencies or hospital collections under Wis. 343.305

Directions for Drawing blood for Implied consent Purposes Wis. 343.305

To the Law Enforcement Agency:

Blood may be withdrawn only by a physician, registered nurse, medical technologist, physician's assistant, or person acting under the direction of a physician. Local enforcement agencies should make suitable arrangements with medical facilities and/or personnel for this purpose. If you transport the subject to a hospital, emergency room, or doctor's office, have this kit available for their use.

 

To the specimen collector:

Under Wis. 343.305 blood specimens may be legally obtained if:

  • The subject has been arrested. 343.305(5)(b)
  • The subject is unconscious. 343.305(3)(b)
  • Directed by a law enforcement officer in conjunction with a lawful OWI arrest, even if the subject refuses the test. State vs Bohling 173 Wis. 2d 529 (1993)

If blood is withdrawn following the above criteria, you and your employer are immune from liability under 343.305(5), provided you are not negligent in performing the procedure (i.e. you use good, accepted technique). This kit should be used for collecting specimens requested by subjects who have been arrested under 343.305. This kit is sometimes used by law enforcement officers for subjects arrested under other statutes.

 

Blood Drawing Instruction:

  1. Use a disposable, sterile needle and holder of your choice. Use the blood collection vials provided in this kit.
  2. Prepare the site using the antiseptic towelette provided in the kit (benzalkonium chloride in water). If the kit towelette is unavailable, you may substitute another non-alcohol based antiseptic, such as povidone-iodine.

  3. Mix blood slowly by inversion several times immediately after drawing.
  4. Place the seal strip over the rubber stopper.
  5. Write the subject's name on the label.
  6. Wrap the label around the tube, covering the ends of the seal strip.
  7. Sign the form enclosed in the kit as to date and time of sampling.
  8. Return the specimen and form to the officer for mailing.

Note: Do not send used needles or towelettes to the laboratory.

 

To the officer:

  1. Fill in all relevant areas (non-laboratory) of the form.
  2. Place the blood tube(s) into the plastic shipping bag along with the absorbent material and seal the bag.
  3. Put the packaged specimen(s) and form in the polystyrene mailing container. Seal the container shut with two bands of tape.
  4. If you wish to have the specimen analyzed at the WSLH, use the label provided; otherwise address the container to the approved laboratory of your choice.
  5. Affix proper postage to the outside of the container.

Kit # 47 - Legal Urine Alcohol

These kits are for law enforcement agencies or hospital collections under Wis. 343.305

    • Blood/Urine Alcohol Analysis sheet
    • Return label with large red seal

  • Desiccant package (absorbent) - Drimop
  • Shipping bag

 

General Instructions:

Note: The Wisconsin State Laboratory of Hygiene requires blood specimens for drug analysis performed under Sec. 343.305(6), Wis. Stats.

The specimen may be collected directly into the container provided in this kit. The arresting officer or his/her designee is responsible for supervising the collection, ensuring that the specimen container is tightly capped, affixing labels and seal strips, completing the form, and mailing or delivering the specimen to the laboratory.

 

Urine collection:

A valid specimen under 343.305(6) Wis. Stat. will be obtained if the following instructions are followed:

  1. Have the subject void (empty bladder) as soon as possible. DO NOT COLLECT THIS SPECIMEN.
  2. Approximately ½ hour later (time not critical), collect a second specimen directly into the container provided in the kit. The subject does not need to fill the container completely. One ounce (30 mL) or about ½ inch of specimen measured from the bottom of the container is sufficient.
  3. Observe the subject during collection to ensure that the specimen is not adulterated or diluted.
  4. Take the specimen container from the subject, screw the cap on tightly and affix the label and seal as directed below.

Specimen Identification, Official Seal, Shipment:

  1. Seal urine container by centering the seal strip over the cap and affixing it to the sides of the container.

  2. Write the subject's name on the label and affix it so that it encircles the container and overlaps both ends of the seal strip. Do not affix the label to the cap.
  3. Complete and sign all applicable sections of the request/report form included in this kit.
  4. Place the specimen container in the plastic biohazard bag along with the absorbent material and seal the bag. Insert the request/report form in the pouch on the OUTSIDE of the biohazard bag.
  5. Place the biohazard bag into ½ of the polystyrene mailing container and place the other half on top of it. Seal the mailing container - two bands of tape across the seams of the box at its ends are sufficient.
  6. Mail or deliver the specimen to an approved laboratory for analysis.
  7. If the specimen is to be mailed to the Wisconsin State Laboratory of Hygiene, use the preaddressed mailing label included in the kit. For other laboratories, indicate the correct address. Affix proper postage and mail.
