Collection Card Ordering and Pricing

The Wisconsin State Laboratory of Hygiene (WSLH) conducts all newborn screening testing in the state. Specimens must be collected directly on a special filter paper card available from the WSLH.

Ordering Collection Cards
To order newborn screening blood collection cards, mail your request for a specified quantity of Kit #213 (newborn screening kits) with your payment to:
