The focus of VeriSamp products is to provide high quality samples which can be used for employee competency, test validation / method correlation, education or to meet regulatory requirements when expanding your laboratory's test menu. VeriSamp products are not a substitute for proficiency testing.
Expected results will be provided for self assessment. Samples may be used until the expiration date when stored according to the directions. New sets are manufactured periodically to provide samples with different results.
Second day shipping is standard and is included in the program price. Express (overnight) shipping is available upon request for an additional charge of $25.00.

VeriSamp Ordering Process
1. Print the PT Plus order form from this website.
2. SL Number: If you are currently or were previously enrolled with WSLH PT, indicate your customer number.
3. Laboratory Contact: Please indicate the name of a contact person.
4. Mailing Address: Please indicate the shipping address for your samples (no PO Boxes, please).
5. Product Request: Indicate the number of sets needed for each product.
6. Check the appropriate Billing Information and Payment Option.

VeriSamp Testing Process
1. Order your samples.
2. Your shipment will include the following:
• Your requested samples
• A page including instructions and space to document your test results
• A sealed envelope containing the sample results
3. Test the samples according to the manufacturer’s instruction of the procedure established in your lab. Most products may be tested two or more times, allowing for testing by multiple personnel or kits.
4. Record the results as you would patient samples.
5. When testing is complete, check results against the exepcted results provided.
6. Document performance.
7. Samples may be used until the expiration date when stored according to the directions.
