What do I need?
Check with your lab's certification agency. Testing/documentation needed prior to reporting patient results vary by accrediting body and/or state. Requirements may include:
a) confirmation of enrollment in an approved PT program
b) 1 or 2 PT challenges
c) correlation studies containing a minimum number of previously tested patient or evaluation samples with known results
d) any combination of the above
What does WSLH PT offer?

- Regularly Scheduled Proficiency Testing –
Program HV for moderate complexity PT which requires 5 samples/event
Program HVQA [waived PT configuration-3 liquid samples/event]. Please note: some certification agencies/states require enrollment in a 5 sample PT program even though the method in use is listed in the “waived” category.

- Off-schedule [“remedial”] PT –
If your lab is required to complete 1 or 2 PT events prior to bringing this test on-line and can’t wait until the next scheduled PT shipment, we offer off-schedule PT which can be shipped within a week of receiving your order.
- Evaluation/correlation/training samples –
VeriSamp product for Anti-HIV testing contains 6 samples. Each set is unique and comes with a sealed envelope listing the results for that set. Please note: these samples are NOT a substitute for proficiency testing.
How do I order?
Current WSLH PT customers can simply call 800-462-5261 to add to their current proficiency testing order.
New customers need to fill out either a WSLH PT Clinical Order Form for proficiency testing programs &/or a WSLH PT Plus VeriSamp Order Form for evaluation/training samples.
